ESA LCA Questionnaire

Description of the questionnaire Tool:

This Questionnaire Tool is intended for data collection from suppliers in order to design a Life-Cycle Assessment model. It is the basis for gathering all necessary information to perform LCA.

This questionnaire is composed by different general tabs named:
1. Start
2. Questionnaire
3. Feedback
4. Export

The Start tab contains the general information in preparation for the questionnaire completion.
The Questionnaire Tab contains all the information to be completed in order to derive an LCA model.

The Questionnaire Tab is split into different sections:
1. Description - Contains the description of the full tab.
2. Project - Shall be filled with Project-related details.
3. Contact Details - Shall be filled with contact details of the user completing the questionnaire.
4. Acronyms - Contains the list of acronyms.
5. Definitions - Contains the list of definitions.
6. Lifecycle Model (product system) - Contains all the necessary fields to create one or more Lifecycle Models, each containing one or more Processes.

Of these, the Questionnaire Tab must be filled in.

Cut-off criteria should be applied to the data, please refer to the Cut Off Criteria Subsection. When needed, some instructions are included in each of the tabs in order to correctly fill in the data. The fields contain an information tooltip, for providing extra information on specific expectations and rules in completing the respective field.

Lifecyle model, Process Block, Inputs, Outputs shall be added through the “+” sign as many times as necessary. In this questionnaire all inputs and outputs should refer to the reference quantity specified in the Functional Unit. Once the Reference functional unit is completed for a Lifecycle Model, a sticky bar appears on the top section, in order to maintain this reference for the user through the completion of the questionnaire. Similarly, once a Process Reference Flow is completed, a sticky bar appears with this information. The inputs and outputs of the process shall be completed with respect to the process reference flow, and the process information, including Process Reference Flow shall be completed with respect to the Lifecycle Model Reference flow.

For copyright considerations, information in this questionnaire is not saved in the background. The information is present as long as the session is open. Your information will not be saved once the session is closed.

In order to save the information introduced, please use the Export functionality in Tab 4.

Required fields shall be completed. The LCA model cannot be performed without these fields, as they contain the minimal accepted information.

Throughout the questionnaire, some data is marked as “import for quality assessment”. In order to derive good quality LCA datasets, this information is expected to be present.

Irrespective of their mandatory/optional nature, you can save at any time all the field contents via the export tab.

At the end of the questionnaire tab, an open field “modelling feedback” is available for comments where the user can present additional considerations or nonconformities with respect to the questionnaire.
